Cal/OSHA's PSM Regulation: Incident Investigation/RCA

Due to recent refinery and process industry accidents, regulators have identified insufficient learning from experience as a contributor in several events. Regulators are currently working to address this apparent shortcoming by updating process safety management (PSM) related regulatory requirements, with Cal/OSHA as the first regulation to be updated.

This webinar will address the following topics:

  • The new requirements for performing an investigation
  • The required hierarchy of controls analysis for each recommendation (note this also applies to recommendations from process hazard analyses, management of change processes, and new installations)
  • Implementation requirements for recommendations from investigations (which apply to recommendations for most elements)

Webinar attendees/personnel responsible for investigations and PSM programs will obtain a better understanding of industry practices and regulatory requirements related to incident investigations.

Lee N. Vanden Heuvel

Lee N. Vanden Heuvel is Сòòò½ÊÓƵ’s lead for incident investigation/root cause analysis (RCA) support services, training and software support. He has assisted organizations in many different industries with the development and implementation of incident investigation and RCA programs. He has also led and participated in investigations in many types of industries, including downstream petrochemical facilities. Vanden Heuvel is a co-author of Guidelines for the Investigation of Chemical Process Incidents (Second Edition), Guidelines for Risk Based Process Safety, and Conduct of Operations and Operational Discipline (all published by the American Institute of Chemical Engineers' Center for Chemical Process Safety), as well as the Root Cause Analysis Handbook (Third Edition). He is also the lead developer of ABS Consulting's RCA software package with partner EXP.
