Human errors may be generally classified as skill-based, decision-based, or perceptual–errors that result in the unsafe act of the operator. These errors committed by frontline personnel are those most closely tied to an accident and may be described as active failures or actions of the operator that result in a human error and unsafe condition. This is the point where most forensic investigations stop. While it may be easy to define and classify human error, it is often difficult to know where to start when looking for error provocative human-system interaction.
